Episodes where Beau is killed - usually violently:

 

To Hell with Babe Ruth - throat slashed by throwing star

Run, Johnny, Run - single gunshot through the head

Ten Thousand Diamonds and a Heart - shot once, uncredited

The Gunrunner - shot down in terrorist gun battle - not sure of his fate

And I want some Candy and A Gun that Shoots - shot by the sniper

Bomb, Bomb, Who's Got the Bomb? - car exploded after going off the road - uncredited

Diary of a Gun - shot by police marksmen after brief crime spree

6,000 Deadly Tickets - security guard shot from, as Che would say, "close range" 

Double Exposure - hit by a car which carried him on the hood and then dumped him over a bridge - dead.

Head to Head - mainland hitman who kills and gets killed at the end

Stringer - burns to death in car blasted off the road

Beau also was shot repeatedly in Savage Sunday but I think it's mentioned he survived.

When he wasn't getting killed by bullets, fire or throwing stars, he was often seen in hand to hand combat.

 

And They Painted Daisies on His Coffin - Beau is one of the beachboys who take on and lose to McGarrett and Kono

The Case Against McGarrett - Beau is the prisoner umpire in the prison rec yard.  When one of the player objects to his (bad) call, Beau decks him and starts a small riot.  (uncredited)

Nine Dragons - costumed as a Nine Dragons member, Beau attacks McGarrett as he's entering the Hong Kong Police Department  (uncredited)

Elegy in a Rain Forest - as Homer Wamano, he exchanges martial arts with Duke

A Short Walk on the Long Shore - as "the knife" he  and another attack the undercover McGarrett.  As usual, his a** is whooped.

Deathmask - a scuffle with fellow stuntman Chuck Couch.  You'd think these two would be doing something like "Bloodsport".

Besides his deaths in cars, he killed someone with a truck (A Bullet for McGarrett and he laughingly crashed a car, surviving (The Clock Struck Twelve) and flipped another while tailing McGarrett (Loose Ends Get Hit).  And he was behind the wheel of the hearse where the coffin falls out (Hookman).
